Andros Fenollosa 5eaeb0202f Rename Categories to Settings, add Fixed Expenses ...

- Rename nav from Categories to Settings with gear icon
- Move all settings URLs under /settings/
- Add FixedExpenseConcept model with CRUD
- Add 14 default fixed expense concepts via seed command
- Settings page shows both categories and fixed expenses sections
- Rename desktop nav Month start to Month
